1. Write to the Honorary Secretary of the Trust,                               Click on ‘contact' to obtain address and e-mail
                               details.

                                      2. Fill in the form answering the questions as
                                         fully as you can.

                                        3. Ask your health visitor, playgroup supervisor,
                                         teacher or someone who knows your child well
                                        to support your application.


                                     4. Return the completed form to the Honorary
                                   Trust Secretary .

                          The Trust will normally meet 3 or 4 times a year to
                     consider applications. The Honorary Secretary will let you
                know the outcome of your application.
